I hereby consent to receive Red Light Therapy (RLT) using the TheraLight 360 Full Body Photobiomodulation System at Fusion Wellness. I understand that RLT involves exposure to low-level red and near-infrared light to support recovery, circulation, tissue repair, skin rejuvenation, and overall wellness. Sessions may also include pulsed electromagnetic field (PEMF) therapy depending on treatment selection.

I understand that the TheraLight 360 is a safe and non-invasive technology, though multiple sessions may be required before results are noticed. Occasionally, mild fatigue, discomfort, or aches may occur after initial sessions — a normal healing response known as "refracing." These symptoms typically resolve within 24–48 hours. If they persist, I will notify the technician prior to my next session.

All persons present during device operation must wear approved TheraLight 360 protective eyewear. Prescription glasses or contacts may be worn under the protective goggles.

I acknowledge that certain medical conditions may make red light therapy unsafe or require additional caution. These include, but are not limited to, pregnancy, active cancer diagnosis, use of immune suppressant medications following organ transplantation, recent steroid injections, seizure disorders, open wounds or skin lesions, and use of photosensitizing medications.

I understand that individuals with seizure disorders should avoid the 100Hz pulse setting and use blackout goggles or a blackout eye pillow during sessions. Clients under 18 years of age must have written parental or guardian consent to participate in TheraLight 360 sessions.

I have disclosed any relevant medical conditions or concerns to Fusion Wellness staff before treatment and agree to notify them of any changes in my health status prior to future sessions.

I acknowledge that while RLT is generally considered safe, potential side effects may include mild warmth, temporary redness, tingling, eye strain, or skin sensitivity. I understand that individual results may vary and that benefits are not guaranteed.

I agree to follow all safety instructions provided by Fusion Wellness staff, including the proper use of protective equipment, and to immediately report any pain, discomfort, dizziness, or unusual sensations during treatment.

I understand that Red Light Therapy is not a substitute for medical diagnosis or treatment, and I am encouraged to maintain regular care with my primary healthcare provider.

I voluntarily assume all risks associated with this therapy and hereby release and hold harmless Fusion Wellness, its owners, employees, contractors, and affiliates from any and all liability, injury, or damages arising from participation in RLT sessions, except in cases of gross negligence or willful misconduct.
